the programme
Referral Agents
How we work with families

For families
to access the programme the main criteria are:
- Mum must
be in her second trimester of pregnancy or Baby must be less than
twelve months old.
- Families
must also reside within the Hamilton City Boundary.
Families
must also fulfill any one of the following criteria:
- Unsupported
parent
- No or
minimal antenatal care
- Young
mother
- Mental
ill health
- Substance
Abuse
- Family
history of abuse
- Relationship
problems
- Low income
status
- Lack
of essential resources
- Frequent
change of address
- Low Maternal
educational qualifications
- SIDS
factors e.g. smoking during pregnancy, little or no breastfeeding, low
birth weight.

Recognised
referral agencies are:
- Lead
Maternity Carers
– Midwives, General Practitioner, Obstetrician
- General
Practitioners
- Hospital
Maternity Services – including neo-natal units
- Well-child
Providers – all providers who are contracted to deliver
the national Well-chilld Tamariki Ora programmes.
- Any
other organisation who is actively working with a family and
recognises the need for a referral to the Family Start Programme

Family Start acknowledges that every family is unique.
With this in mind, Family Start has developed three distinct approaches
to working with families; these approaches are based on the intensity
of the needs that are identified by the family.
Teaming
Teaming is
made up of at least three Kaitiaki and will spend 4 hours a week working
with families whose needs are high, complex and intense. Teaming is a
short-term crisis intervention team who will work with the family until
the family feel that they no longer require the services of Teaming.
Co-working
Co-working
is made up of two Kaitiaki (one male and one female Kaitiaki) and will
spend 2.5 hours a week working with families whose needs are of medium
intensity; Co-working will continue to do so until the family feel that
they no longer require the services of Co-working.
Families who were previously working with Teaming may wish to be moved
into Co-working.
Individual
Support
Individual
Support involves one Katiaki who will spend 2.5 hours per fortnight working
with families whose needs are of low intensity.
Families who were previously working with Co-working may wish to be moved
into Individual Support.
Families who work with Individual Support are often nearing graduating
and exiting from the Family Start programme.
